informed consent in dementia trial

hy all,
 i have a que....i tried to search it in the forum but was not able to find it........
in a trial for the disease dementia where the patients mental condition keeps on decreasing........can we take informed consent from the patient again for the second tym......suppose at starting patients consent was taken but after some tym the patient mental condition is not lyk before so can we take the inform consent again from the patient......

Hello Vyom

In case of Dementia trial we dont take CONSENT of the subject but LAR gives consent on behalf of Subject. In this kind of trial Subject gives assent.

in this case LAR will give his consent for the trial & consent is voluntary so at any point LAR can chg decision 
the responsibilty goes to the PI that the study shd b explained to the pt up to his level of understanding

 If LAR is not taking responsibility , the subject will be dropped in this case.

I meant that the PI shd take efforts to chk whether LAR has explained / given max info to sub
